JACKSONVILLE, Fla. – The Jacksonville Sheriff’s Office released body cam footage of an officer-involved fatal shooting of a 71-year-old man on July 19.

Officers responded to a crash with injuries on Beach Boulevard at the intersection of the I-295 Northbound ramp. They learned that the man had run the red light and hit another vehicle driven by a mother and her two children in the back seat.

They weren’t seriously injured, but both vehicles were heavily damaged.

JSO said the man gestured a double thumbs-up to the other driver and got out of his car with a handgun. Jacksonville Fire and Rescue arrived on the scene and said that the man said he wanted to shoot himself in the head.

JSO officers Greg Hernandez and Zacharey Slatowski arrived and joined the other patrol officers.

JSO said the man was seated on a construction trailer in a grassy area near the crash site with a handgun between his legs. Hernandez and Slatowski armed themselves as they negotiated with him for 10 minutes, directing him to disarm himself.

During the negotiation, JSO said a bicyclist entered the restricted area and they yelled at him to leave the scene.

Officials said the man then stood up and began walking, still armed, toward a crowd of people standing on a hotel property on Beach Boulevard.

Hernandez and Slatowski can be seen on the body cam video firing their rifles at the man. JFRD began medical care and transported him to a nearby hospital, where he died.

This is the 12th officer-involved shooting in 2025.
